Medical Statement for TDI Technical Diving

The purpose of this medical questionnaire is to find out if you should be examined by your doctor before participating in technical diver training. A positive response to a question does not necessary disqualify you from diving. A positive response means that there is a pre-existing condition that may affect your safety while diving and you might have to seek the advice of your physician.

Please click on the left icon below and read the Medical Statement. If you find you must answer yes to any question, print out both the Medical Statement and the Physician's Release and bring them to your doctor or preferably to a diving physician.
