Medical Statement for Scuba Diving
The purpose
of this medical questionnaire is to find out if you should be examined
by your doctor before participating in recreational diver training. A
positive response to a question does not necessary disqualify you from
diving. A positive response means that there is a pre-existing condition
that may affect your safety while diving and you might have to seek the
advice of your physician.

you must answer yes to any question, print out the whole statement including
the attached "Guidelines for Recreational Scuba Divers' Physical Examination"
and bring it to your doctor or preferably to a diving physician.
