Medical statement for scuba diving
Before enrolling on any PADI or DSAT course, you must complete and sign a medical self-declaration. Please download and complete the file attached below, then bring it with you when enrolling on your course.
If you answer ‘yes’ to any question on the medical self-declaration:
Before arrival, you must consult your doctor (preferably one who is specialised in diving medicine). To enroll on your course, you will need to bring a letter giving you medical approval to dive, as well as your completed declaration. If you answer ‘yes’ to any question on the medical self-declaration, but do not bring a letter of medical approval, we may refer you to a specialist diving doctor in Sharm el Sheikh (cost approx. Euro 35.00). In this case, you will need to join a later course (subject to availability) instead of the one you have booked.
